In recent decades, there has been a growing recognition of the importance of social determinants of health (SDH) in shaping individual and population health outcomes.1 Factors such as socioeconomic status, edu[1]cation, housing, and social support networks are critical in influencing physical and mental health, yet they often fall outside the remit of traditional healthcare systems.1,2 Addressing these determinants requires holistic approaches that integrate medical and social care interventions.3 One such approach is social prescribing (SP), which connects individuals with community resources to support their health and wellbeing.
